Do I really have to fill in the back of the form?

Yes, this is the most important part of the audit – the information provided on the reverse of the form is entered into a completely different computer pathway (patient de-identified). We retain that information for you, run the program once all data for the report time period is due and then all the doctor data included is compared and statistical graphs and percentages (%) are produced.
